Prescription & Script Requests: IMPORTANT INFORMATION

Before submitting a request, please read the following reminders:

• E-Scripts
  All prescriptions will be sent via e-script directly to the mobile number you have provided. Please ensure the number you submitted is current and active.

• ADHD Medications (Ritalin, Methylphenidate Hydrochloride, Concerta, Dexamfetamine, etc.)
  A review appointment is required before a repeat prescription can be issued.

• Leucovorin
  A review/follow-up appointment is required for prescription requests.

• Intuniv
  You may request a prescription via the link only if your child has had an appointment within the last 3 months. If not, a review/follow-up appointment is required.

• CBD Medications
  You may request a prescription via the link only if your child has had an appointment within the last month. If not, a review/follow-up appointment is required.

• Forms & Reports (e.g. Carer Allowance Form)
  Completion of forms by the doctor/paediatrician requires a 15-minute appointment. Please email us at reception@leilamasson.com to book an appointment.
